For health insurance, Ocala residents without a job-based plan buy coverage on the federal Marketplace at HealthCare.gov. Open Enrollment for 2027 runs Nov 1, 2026 – Jan 15, 2027. Adults in their late 50s and early 60s and self-employed horse-industry workers often benefit most from a premium tax credit, which depends on 2027 income.

Health insurance Ocala: Marion County at a glance
Ocala is the county seat of Marion County, and two things set it apart from most Florida cities. First, the county is older than average: U.S. Census Bureau QuickFacts puts the share of residents 65 and over at 29.7% (2025 estimate). Second, the local economy runs on horses.
A 2023 American Horse Council Foundation study released by the Florida Thoroughbred Breeders’ and Owners’ Association credits Marion County, the trademarked “Horse Capital of the World,” with about 75,000 horses and ponies and roughly 28,500 equine-related jobs, about one-fifth of county employment. Many of those jobs are on small farms or self-employed, with no group health plan.
Self-employed in horse country
Trainers, grooms, farriers, exercise riders, hay and feed suppliers, and boarding-barn owners often work for themselves or for operations too small to offer coverage. For these workers, the Marketplace is usually the main source of health insurance. Ocala riders and farm owners can buy the same plans as anyone else, and insurers can’t turn you down or charge more for a pre-existing condition, including an old riding injury.
Estimate net income, not gross
Your premium tax credit is based on household income after business expenses. If you own a farm with feed, vet, and equipment costs, your net self-employment income may be far below your gross receipts. Use your best estimate of 2027 net income on the application, and update it if the sales season changes your numbers. HealthCare.gov’s self-employed guide explains how to report it.
Seasonal and uneven pay
Income in the equine business can swing with sale seasons and show schedules. If yours changes during the year, report the change so your tax credit stays accurate and you’re not surprised when you file your 2027 return in 2028. Our self-employed health insurance guide covers deductions and plan choices.

Coverage in the years before Medicare
Many Ocala residents retire, sell a business, or leave a job in their late 50s or early 60s. That creates a gap: employer coverage ends, but Medicare doesn’t start until 65. A Marketplace plan is built to fill it, and losing job-based coverage opens a 60-day Special Enrollment Period.
Age matters more here than anywhere else in the Marketplace. Under federal rules, premiums can be up to three times higher for older adults than for younger ones, according to HealthCare.gov. Because the subsidy caps what you pay toward the benchmark silver plan as a share of income, a 62-year-old often gets a far larger credit than a 30-year-old with the same income.
Watch the 400% cliff. For 2027 coverage, a two-person household earning more than $86,560 gets no premium tax credit. Pre-Medicare couples near that line should plan IRA withdrawals, capital gains, and part-time pay carefully. Social Security benefits count too, so get help before you apply.
The Medicare handoff is the other big question about health insurance Ocala retirees ask. When you reach 65, your Marketplace plan doesn’t end on its own. Once you qualify for premium-free Medicare Part A, you can’t keep a premium tax credit, so cancel your Marketplace plan to line up with your Medicare start date. HealthCare.gov explains the switch. A younger spouse can stay on the Marketplace plan.
What you’ll pay toward the benchmark plan in 2027
For 2027 coverage, the IRS sets the share of income you’re expected to pay for the benchmark silver plan. The tax credit covers the rest. Because Florida didn’t expand Medicaid, subsidies for health insurance Ocala residents buy start at 100% of the poverty level.
| Household income (% of poverty level) | Expected share of income for benchmark silver |
|---|---|
| 100% – 133% | 2.15% |
| 133% – 150% | 3.23% – 4.30% |
| 150% – 200% | 4.30% – 6.78% |
| 200% – 250% | 6.78% – 8.66% |
| 250% – 300% | 8.66% – 10.22% |
| 300% – 400% | 10.22% |
| Over 400% | No subsidy |
For one person, 100% of the poverty level is $15,960 and 400% is $63,840. The 2027 out-of-pocket maximum is $12,000 for an individual and $24,000 for a family. KFF found insurers proposed a median 15% increase for 2027, so compare plans instead of auto-renewing. See our 2027 subsidies guide for the full table.
Hospitals and networks in Ocala
Ocala has several hospital systems, so network checks matter. According to HCA Florida Healthcare, the trauma center at HCA Florida Ocala Hospital and HCA Florida West Marion Hospital is verified as a Level I trauma center by the American College of Surgeons, and the two hospitals list 585 licensed beds. AdventHealth Ocala is the city’s other large hospital, and UF Health runs UF Health Ocala Neighborhood Hospital.
No single plan is guaranteed to include all of them. When you compare health insurance, Ocala residents should check networks first. Before you pick a 2027 plan, confirm that your doctors, specialists, preferred hospital, and prescriptions are in-network. Carriers and networks vary by county, and 2027 lists for Marion County usually aren’t final until late October. Frequently asked questions
I’m 61 and retired early in Ocala. What are my options?
A Marketplace plan is the most common bridge to Medicare. Losing job-based coverage opens a 60-day Special Enrollment Period, and your premium tax credit is based on your estimated 2027 household income, so planning retirement withdrawals can affect how much help you get.
How do self-employed horse-industry workers get health insurance?
Most buy a Marketplace plan. Your subsidy is based on net self-employment income after business expenses, not gross receipts. If your income changes with sale or show seasons, report it during the year so your tax credit stays accurate.
What happens to my Marketplace plan when I turn 65?
It doesn’t end automatically. Once you qualify for premium-free Medicare Part A you can no longer get a premium tax credit, so cancel the Marketplace plan to line up with your Medicare start date. A younger spouse can keep Marketplace coverage.
Is there in-person enrollment help in Marion County?
Marion County’s federal Navigator help is virtual-only for 2026–2027, so assistance is by phone or video. Search localhelp.healthcare.gov for assisters, or get help from a licensed agent by chat, voice, or text.
When is Open Enrollment for 2027 coverage in Ocala?
Open Enrollment runs November 1, 2026 through January 15, 2027. Enroll by December 15, 2026 for coverage starting January 1, 2027. Outside that window you need a qualifying life event, such as losing other coverage.
