Health insurance Utah 2027 at a glance
Utah residents who don’t get health insurance through work, Medicare or Medicaid buy their own plan on HealthCare.gov. The state doesn’t run its own exchange, so federal rules set the enrollment window and the subsidy formula. The table below is the short version; every row is explained further down.

|---|---|
| Exchange | HealthCare.gov (federal platform) |
| Open Enrollment | Nov 1, 2026 – Jan 15, 2027 (Dec 15 for Jan 1 coverage) |

| New Medicaid rule | Community Engagement Requirement for expansion adults starts Jan 1, 2027 |
| Children’s coverage | Utah CHIP (and Medicaid for kids) |
| Where to apply for Medicaid/CHIP | Department of Workforce Services, jobs.utah.gov |
| Individual-market insurers | Six insurers filed for 2027, all returning from 2026 |

How the Utah Marketplace works
The Marketplace is the only place Utahns can use an income-based premium tax credit. Every plan sold there covers the ten essential health benefits, including doctor visits, hospital care, prescriptions, maternity and mental health care, and no plan can turn you down or charge more for a pre-existing condition.
You can enroll directly on HealthCare.gov or through a licensed agent or enrollment assistant connected to it. The plans and prices are the same either way. Plans come in Bronze, Silver, Gold and Platinum tiers, plus Catastrophic plans for people under 30 or with a hardship exemption.
New to the system? Our explainer on the health insurance Marketplace covers metal tiers, networks and how premiums and deductibles trade off.
2027 Open Enrollment dates for Utah
Because Utah uses HealthCare.gov, Open Enrollment for 2027 coverage runs from November 1, 2026 to January 15, 2027. Enroll by December 15 and coverage starts January 1; enroll between December 16 and January 15 and coverage starts February 1.
A 2025 federal rule would have ended the window on December 15, but a federal court blocked that cutoff and CMS confirmed the January 15 end date for this season in an August 2026 statement. The practical advice doesn’t change: treat December 15 as your real deadline so there is no gap on January 1.
Missed the window? A move, job loss, marriage, a new baby or losing Medicaid can open a Special Enrollment Period, usually for 60 days. Full details are in our Open Enrollment 2027 guide.
Subsidies and the 400% cliff in 2027
The enhanced premium tax credits that ran from 2021 through 2025 expired on December 31, 2025, and they were not extended. For 2027 coverage, the original credit applies: it starts at 100% of the federal poverty level and stops at 400%. In Utah, most adults under 138% go to Medicaid instead.
The credit caps what you pay for the benchmark Silver plan at a share of income, from 2.15% at the low end to 10.22% between 300% and 400% of poverty, under IRS rules. Above 400%, which is $63,840 for one person or $132,000 for a family of four, there is no credit at all.
Utah tip: Utah families are larger than the national average, according to Census QuickFacts. Each extra person raises the 400% line by $22,720, so a bigger household can qualify at an income that would be too high for a smaller one. Count everyone you claim on your 2027 tax return.
Insurers nationally proposed a median 15% increase for 2027, according to KFF. That makes shopping more important than ever. Our 2027 subsidies guide has the full applicable-percentage table and the new immigrant eligibility limits that begin January 1, 2027.

Utah Medicaid, the work requirement and CHIP
Utah fully expanded Medicaid to adults with income up to 138% of the federal poverty level on January 1, 2020, after a partial expansion in 2019, according to Utah Medicaid. For one adult that line is about $22,025 for 2027 eligibility. Adults under it should apply for Medicaid rather than a Marketplace plan.
The Community Engagement Requirement starts January 1, 2027
Starting January 1, 2027, adults ages 19 to 64 in Adult Expansion and Targeted Adult Medicaid must show 80 hours a month of work, job training, school at least half-time or volunteering, or meet a minimum monthly earnings level, per the state’s Community Engagement Requirement page. Pregnant and postpartum members, American Indian and Alaska Native adults, certain caregivers, people who are medically frail and some others are exempt.
Utah also moves expansion adults to renewals every six months in 2027. If you lose Medicaid because of the new rules or a missed renewal, that loss can open a Special Enrollment Period for a Marketplace plan, and with income above 100% of poverty you may qualify for a premium tax credit.
Watch your mail. Utah sent Community Engagement notices in July 2026. Keep your address current with the Department of Workforce Services so you don’t miss a renewal or proof request.
Children: Utah CHIP
Children in families that earn too much for Medicaid may qualify for Utah CHIP. You apply for Medicaid and CHIP through the Department of Workforce Services (myCase), and you can apply any time of year.
Insurers selling 2027 individual plans in Utah
Six insurers filed to sell individual plans in Utah for 2027, and all six sold plans in 2026: BridgeSpan, Regence BlueCross BlueShield of Utah, SelectHealth, University of Utah Health Plans, Imperial Health Plan of the Southwest and Molina Healthcare of Utah, according to ACASignups’ tracking of Utah filings. Final average increases there were about 7.4%, well under the national median.
Not every insurer sells in every county. SelectHealth filed to offer plans statewide, while others cover a narrower area, so the list you see depends on your ZIP code. Cigna left Utah’s individual market for 2025 and Aetna left the ACA market after 2025, so neither is an option for 2027. Rate filings are public on the Utah Insurance Department’s health rates site.
Networks matter more than logos. Intermountain Health, University of Utah Health and MountainStar hospitals sit in different plan networks, so check that your doctors are in-network for 2027 before you choose. Our guide on how to choose a health plan explains HMO, EPO and PPO differences.
Salt Lake City, Provo and the rest of Utah
Health insurance Utah rules are statewide, but local hospitals, clinics and jobs change the best choice. We built city guides for Utah’s two biggest search markets:
- Health insurance in Salt Lake City: Salt Lake County clinics, hospital financial assistance, University of Utah students and earthquake-preparedness tips.
- Health insurance in Provo: Utah County’s large young families, BYU and UVU student coverage rules, and the Mountainlands community health center.
Outside those metros, from Ogden and Logan to St. George, the same HealthCare.gov plans apply, but the number of insurers in your county may be smaller. Always compare by ZIP code.
Free help and where to complain in Utah
Utah has no federal Navigator for 2026–2027: CMS’s Navigator awardee list notes the state’s only awardee withdrew in August 2026. Free in-person help is still available from Take Care Utah, whose Health Access Assisters help with Marketplace, Medicaid and CHIP applications, or by dialing 2-1-1.
For a claim denial, billing problem or a question about your insurer, contact the Utah Insurance Department. Its health consumer line is 801-957-9280. You can also search localhelp.healthcare.gov for certified assisters and agents near you.
Utah’s uninsured rate was 8.3% in 2024, close to the national rate, according to America’s Health Rankings using Census American Community Survey data. Many of those residents qualify for a subsidy or Medicaid and don’t know it.
Five health insurance Utah mistakes to avoid in 2027
Most coverage problems we see come from a handful of avoidable habits, not from bad luck. Each one below is easy to fix during Open Enrollment, and fixing it can protect both your budget and your access to the doctors you already trust.
- Letting your plan auto-renew. HealthCare.gov can re-enroll you, but your 2026 plan’s premium, network or drug list may change for 2027. Health insurance Utah shoppers who compare usually find a better fit than the one they rolled into.
- Guessing at income. Your credit is based on estimated 2027 household income and reconciled on your 2027 tax return. Report raises, side work and new jobs during the year so you don’t repay a large amount.
- Ignoring the 400% line. A small raise can push a household past the subsidy cliff. Contributions to a traditional 401(k) or HSA lower the income the Marketplace counts.
- Skipping the network check. Utah’s big health systems are split across different plan networks. Search every doctor, hospital and prescription before you enroll.
- Missing Medicaid mail. With six-month renewals and the new work rule, a missed letter can end coverage. If it does, you have a limited window to switch to a Marketplace plan.
Good health insurance Utah decisions start with the numbers that matter for your household: income, the people on your tax return, your doctors and the medicines you take every month.
