How the Louisiana Marketplace works

Louisiana does not operate its own exchange, so every individual and family plan sold with a premium tax credit goes through HealthCare.gov. You can apply on the website, by phone, with a Navigator, or through a licensed agency connected to the federal platform. The plan price is the same whichever door you use.

The Marketplace is the right place for you if you do not get affordable coverage from a job, are not on Medicare, and earn too much for Medicaid. That describes a lot of Louisianans: offshore and plant contractors, restaurant and hospitality workers, fishermen, farmers, early retirees, rideshare drivers and small-business owners.

Demand is real. CMS counted 296,648 Louisiana plan selections on HealthCare.gov during the 2026 Open Enrollment Period, according to the CMS 2026 Open Enrollment report. Every plan covers the ten essential health benefits, and no insurer can turn you down or charge you more for a pre-existing condition. For the national picture, read our explainer on the health insurance Marketplace.

Open Enrollment dates for 2027 coverage

Louisiana follows the federal calendar. Open Enrollment runs from November 1, 2026 through January 15, 2027, and the date you finish your application decides when coverage starts.

If you enroll…Coverage starts
Nov 1 – Dec 15, 2026January 1, 2027
Dec 16, 2026 – Jan 15, 2027February 1, 2027
After Jan 15, 2027Only with a Special Enrollment Period

Treat December 15, 2026 as your real deadline. A federal court blocked a rule that would have ended Open Enrollment on Dec 15, and CMS confirmed the Jan 15 end date for this season, but finishing early protects you from any gap on New Year’s Day. Our Open Enrollment 2027 guide covers renewals and plan changes.

Outside Open Enrollment, you need a qualifying life event such as losing other coverage, moving, marriage or a new baby. Louisiana’s hurricane seasons matter here too: if FEMA declares a disaster in your parish and it kept you from enrolling, HealthCare.gov allows a Special Enrollment Period for 60 days after the incident period ends. See our Special Enrollment Period guide.

Medicaid, Healthy Louisiana and LaCHIP

Louisiana expanded Medicaid on July 1, 2016, so adults 19 to 64 with income up to 138% of the federal poverty level can qualify. You can apply any time of year; there is no enrollment window for Medicaid.

Most members get their benefits through a Healthy Louisiana managed care plan. One change matters for 2027: the Louisiana Department of Health says the Healthy Blue contract ends December 31, 2026, and its roughly 290,000 members move to the four remaining plans on January 1, 2027. If you are a Healthy Blue member, watch your mail and check that your doctors take your new plan.

Children under 19 can be covered by LaCHIP at no cost to the family when household income is within program limits. LDH also runs the LaCHIP Affordable Plan for families a little above those limits. You apply for Medicaid and LaCHIP online at the LDH self-service portal (MyMedicaid.la.gov) or by calling Louisiana Medicaid at 1-888-342-6207.

Tip: If HealthCare.gov says you may qualify for Medicaid, your application is sent to Louisiana Medicaid for a decision. Answer any LDH request for documents quickly so your file doesn’t stall.

Louisiana Medicaid work requirements from 2027

Starting January 1, 2027, many expansion adults must show work or another qualifying activity to keep Medicaid. LDH says the rule applies to new applicants and to people at renewal.

According to LDH’s work requirements page, adults 19 to 64 generally need 80 hours a month of work, volunteering or a work program, or monthly earnings at a set level, or half-time school enrollment. Exempt groups include parents of a child under 14, pregnant and postpartum members, people who are disabled or medically frail, and veterans with a total disability rating. LDH also says retroactive coverage shrinks to one month for most adults from January 1, 2027.

If you lose Medicaid because you do not meet the requirement, check the Marketplace right away. Losing Medicaid is a qualifying event, and depending on your income you may be eligible for a premium tax credit. We cannot predict individual Medicaid decisions, so always read LDH notices and keep copies of what you send.

Subsidies and costs for health insurance Louisiana shoppers

The premium tax credit is still available for 2027, but it is smaller than it was through 2025. The enhanced credits expired December 31, 2025, so households above 400% of the poverty level get no credit in 2027, and everyone else pays a larger share of income for the benchmark Silver plan.

Because Louisiana expanded Medicaid, Marketplace credits generally begin at 138% FPL; below that, adults are usually routed to Medicaid. For 2027 coverage, a household between 300% and 400% of FPL is expected to pay 10.22% of income for the benchmark Silver plan, under IRS premium tax credit rules and Rev. Proc. 2026-26. Silver plans with cost-sharing reductions still lower deductibles for incomes up to 250% FPL.

Rates are rising again. LDI approved final 2027 individual-market rates in September 2026, and the ACASignups summary puts the average increase at 18.1% across six carriers. The 2027 out-of-pocket maximum is $12,000 for one person and $24,000 for a family. Insurers and networks in Louisiana for 2027

Six insurers filed 2027 individual-market plans that LDI approved, but not every carrier sells in every parish. Your ZIP code decides which plans you see on HealthCare.gov.

When you compare health insurance, Louisiana networks matter as much as the monthly premium. Final 2027 plan listings appear when window shopping opens on HealthCare.gov, usually in late October. Until then, carriers and networks vary by parish, so compare every option for your address rather than relying on last year’s list. Networks are often built around specific hospital systems such as Ochsner Health, LCMC Health or Franciscan Missionaries of Our Lady Health System (Our Lady of the Lake). Before you choose, search each plan’s directory for your doctors, your hospital and your prescriptions.

If you already have a 2027 plan offer in the mail, don’t auto-renew without shopping. Prices and networks change every year, and a plan that worked in 2026 may not be the best fit for 2027. Our checklist on how to choose a health plan explains HMO, EPO and PPO differences.

Louisiana city guides

Local details change how you shop for health insurance. Louisiana parishes differ. Hospital networks, clinics and parish programs look very different in Baton Rouge than in New Orleans, so we built city guides for the two places Louisianans search most.

  • Health insurance in Baton Rouge, LA: state-employee and LSU households, Our Lady of the Lake and Baton Rouge General networks, CareSouth and other community health centers, and flood-season enrollment tips for East Baton Rouge Parish.
  • Health insurance in New Orleans, LA: Ochsner and LCMC networks, hospital charity care, community clinics such as DePaul and St. Thomas, and coverage advice for hospitality and gig workers in Orleans Parish.

Free help and consumer protection in Louisiana

You do not have to do this alone. Free, unbiased help is available statewide, and the state insurance department handles complaints about insurers.

  • Navigator: the CMS 2026–2027 Navigator awards name the Southwest Louisiana Area Health Education Center as Louisiana’s statewide Navigator. Search localhelp.healthcare.gov for appointments near you.
  • Louisiana Department of Insurance: LDI answers questions and takes complaints at 1-800-259-5300.
  • Louisiana Medicaid: 1-888-342-6207 for Medicaid and LaCHIP applications and renewals.

Coverage still matters here. The Census Bureau reports Louisiana’s uninsured rate rose to 7.7% in 2024 from 6.9% in 2023, with working-age adults at 11.5% (Census ACS brief).
