North Carolina health insurance helps a family at home compare 2026 Marketplace plans

North Carolina Health Insurance 2027: Marketplace Plans, Quotes, Costs & Subsidies

North Carolina health insurance for 2027 is sold through the federal HealthCare.gov Marketplace, where income-based subsidies lower your monthly premium. Many residents now qualify for free or low-cost NC Medicaid after the state expanded coverage in December 2023. Compare every plan with Honesty.AI to find the right fit for your family.

Updated for the 2027 plan year (October 2026). Enrollment and premium figures are from 2026 and are typical examples, not quotes.

How health insurance works in North Carolina

North Carolina uses the federal HealthCare.gov Marketplace rather than running its own state exchange. You shop, compare plans, apply your subsidy, and enroll all in one place. For the 2026 plan year, 761,457 North Carolinians selected a Marketplace plan, choosing among products from six private insurers. For 2027, five insurers have filed to sell Marketplace plans, according to the NC Department of Insurance.

Every Marketplace plan covers the same ten essential health benefits, including doctor visits, hospital care, prescriptions, maternity, and mental health. Plans differ in monthly premium, deductible, and network, not in whether they cover the basics. That is why comparing total yearly cost, not just the premium, matters so much.

If your income is low enough, you may skip Marketplace premiums entirely and qualify for NC Medicaid. Learn more about the broader system in our guide to the health insurance Marketplace. If cost is your biggest worry, start with our guide to affordable health insurance in North Carolina.

Who each coverage type is for

There is no single right plan. The best coverage depends on your household income, health needs, and how you prefer to get care. Honesty compares ACA, alternative, and ancillary plans so you see the full picture, not just one carrier’s shelf.

ACA Marketplace plans

Best for most individuals and families who want comprehensive coverage with subsidy help. These plans cannot deny you for pre-existing conditions and include all essential benefits.

Alternative plans

May suit healthy people who need a temporary bridge or a lower monthly cost and understand the trade-offs. These are not ACA plans and may not cover pre-existing conditions, so read the fine print carefully.

Ancillary plans

Dental, vision, accident, and similar add-ons that fill gaps around a main medical plan. They work alongside, not instead of, major medical coverage.

NC Medicaid expansion: who now qualifies

This is the biggest recent change for North Carolina health insurance. On December 1, 2023, North Carolina became the 40th state to expand Medicaid, opening free or low-cost coverage to adults who earned too much for old Medicaid but too little for strong Marketplace subsidies. More than 600,000 residents gained access in the first months.

You may now qualify for NC Medicaid if you are age 19 to 64, a North Carolina resident, a U.S. citizen or qualifying immigrant, and your household income is at or below 138% of the federal poverty level. There is no asset test, so you can own a home, a car, and savings and still qualify.

Current income guide (138% FPL under the 2026 poverty guidelines, expansion adults): roughly $22,025/year for one person, $37,702/year for a family of three, and $45,540/year for a family of four. Income near these limits should still apply, since rules can shift.

Medicaid enrollment is open year-round, not just during Open Enrollment, so you can apply the moment you qualify. You can confirm current rules at the official NC Medicaid expansion page. If your income is just above the Medicaid line, Marketplace subsidies likely pick up where Medicaid stops.

New for 2027: starting January 1, 2027, most expansion adults ages 19 to 64 without a disability must show 80 hours a month of work, school, or volunteering, or monthly earnings equal to at least 80 hours at the federal minimum wage, unless exempt, and will renew every six months instead of yearly, according to NCDHHS. Losing Medicaid for not meeting this rule also blocks Marketplace subsidies, so respond to every notice.

2027 costs & subsidies in North Carolina

Marketplace premiums depend on your age, county, plan tier, and tobacco use, but subsidies change what you actually pay. The subsidy is an advance premium tax credit based on income, and it is applied directly to your monthly bill. Many North Carolinians pay far less than the sticker price.

One important change still shapes 2027: the enhanced premium tax credits that boosted savings in recent years expired at the end of 2025 and were not extended. Subsidies still exist, but they are smaller, and the 400% federal poverty level cliff applies to 2027 coverage. KFF found that subsidized out-of-pocket premiums more than doubled on average last year, and North Carolina insurers have requested 2027 increases ranging from about 8.5% to 27.4%, according to the NC Department of Insurance’s initial filings. Final rates come after state review, so comparing plans carefully matters more than ever.

Never assume you earn too much to qualify. With the 400% cliff back, a small income change can flip your subsidy on or off. Run your real numbers before deciding you cannot afford a plan.

Getting North Carolina health insurance quotes

A real North Carolina health insurance quote needs five things: your ZIP code and county, the ages of everyone who needs coverage, tobacco use, your expected 2027 household income and the doctors and prescriptions you want covered. Without income, a quote shows only the full price; with it, you see what you would actually pay after the premium tax credit.

Insurers price individual plans by age, location and tobacco use only. They cannot charge more or turn you down because of a health condition, so you don’t need to answer medical questions to get a Marketplace quote. If a website or caller asks about your health history before quoting, you are probably being offered something other than an ACA plan.

  • ZIP code and county: carriers and networks vary by county.
  • Birthdates: premiums rise with age, and children are priced separately.
  • Expected 2027 income: drives your subsidy and any cost-sharing reductions, and tells you if NC Medicaid fits better.
  • Doctors and prescriptions: so you can filter for plans that include them.
  • Any job-based coverage offer: an affordable offer can block your credit.

Quotes for 2027 coverage become final when HealthCare.gov opens window shopping before November 1. Because insurers requested 2027 increases, re-quote even if you plan to keep the same plan; automatic renewal can leave you in a plan that no longer fits your budget.

Metal tiers explained

Marketplace plans come in metal tiers that describe how you and the plan split costs, not the quality of care. A higher tier means a higher premium but less you pay when you need care.

TierPlan pays ~Best for
Bronze60%Lower premium, higher out-of-pocket; healthy and rarely at the doctor
Silver70%Balanced; unlocks cost-sharing reductions if income qualifies
Gold80%Frequent care or ongoing conditions
Platinum90%Highest premium, lowest cost at care

Catastrophic plans are also available for people under 30 or those with a hardship exemption. If your income is modest, a Silver plan can be the smartest pick because it unlocks extra cost-sharing reductions that Bronze and Gold do not.

When you compare North Carolina health insurance tiers, think about your real year. Add up your expected premiums plus the deductible and copays you are likely to hit, not just the lowest monthly price. A cheap Bronze plan can cost more overall if you see doctors often, while a Gold plan can save money for a family that uses care regularly.

Networks, counties, and carrier choices

North Carolina health insurance is sold county by county, so the carriers and networks available to you depend on where you live. Six insurers offered Marketplace plans in 2026, and five have filed to offer plans for 2027, which makes checking your own ZIP code important before you choose.

Each plan has a provider network of doctors and hospitals. Before enrolling, confirm your current doctors and preferred hospital are in-network, and make sure your regular prescriptions are on the plan’s drug list. A low premium is not a bargain if your family has to switch providers or pay full price for medications.

Keep a short list of your must-keep doctors and prescriptions handy. Honesty can check them against every plan at once so you are not calling offices one by one.

Local options matter too. Our city guides cover hospital networks, clinics and free enrollment help in Charlotte and Greensboro.

Is your insurer leaving for 2027? Cigna is ending its ACA Marketplace plans after December 31, 2026, and North Carolina is one of the 11 states affected. Cigna members in North Carolina should compare the remaining insurers and pick a 2027 plan by December 15, 2026 for a January 1 start.

Best individual health insurance in NC: how to judge “best”

The best individual health insurance in NC is the plan that covers your doctors and medicines at the lowest total yearly cost for your situation, and that differs from family to family. No insurer is best for everyone, and rankings that ignore your county, income and prescriptions are not much help.

For 2027, five insurers filed individual-market rates with the NC Department of Insurance: Blue Cross and Blue Shield of North Carolina, Ambetter of North Carolina, Oscar, UnitedHealthcare and AmeriHealth Caritas. Use these tests to compare their plans in your county:

  1. Network: are your primary care doctor, specialists and preferred hospital in it?
  2. Drug list: are your prescriptions covered, and at what tier?
  3. Metal tier: does the deductible fit how often you use care?
  4. Cost-sharing reductions: at or below 250% of poverty, an enhanced Silver plan often beats Bronze and Gold.
  5. Total cost: premium after subsidy plus the deductible and copays you expect, up to the 2027 out-of-pocket maximum of $12,000 for one person or $24,000 for a family.

Private health insurance in NC bought directly from an insurer “off exchange” follows the same ACA rules, but it can’t use the premium tax credit. If you might qualify for a subsidy, compare on HealthCare.gov or with a licensed agent who can apply it.

Short-term health insurance in North Carolina

Short-term health insurance in North Carolina is a temporary, non-ACA policy, and state law ties it to the federal definition. In Bulletin 25-B-11 (August 29, 2025), the NC Department of Insurance said it will keep following the current federal rule until a new one is finalized, even though federal agencies announced they would not prioritize enforcing it.

Our short-term health insurance guide compares these plans with ACA coverage side by side and covers the rules in other states.

That federal rule, adopted in 2024, limits short-term plans sold since September 1, 2024 to an initial term of no more than three months and no more than four months in total, including renewals. As of October 2026, federal agencies have said they plan to rewrite it, but no replacement has been finalized, so the three- and four-month limits still apply in North Carolina.

Know the trade-offs: short-term plans can refuse to cover pre-existing conditions, don’t have to cover all essential health benefits, can cap benefits and can’t use premium tax credits. They are not a substitute for an ACA plan if you have a health condition, take regular medicine or are pregnant.

A short-term plan can bridge a brief gap, such as a few weeks between jobs, for someone healthy. Before buying one, check whether you have a Special Enrollment Period, whether NC Medicaid fits your income, or whether COBRA makes sense. Honesty compares ACA, alternative and ancillary plans side by side, so you can see the gaps before you decide.

How to enroll in North Carolina health insurance

Enrolling is straightforward when you know the windows. For 2027 coverage, Open Enrollment runs November 1, 2026 through January 15, 2027. Enroll by December 15, 2026 for coverage starting January 1, 2027, or December 16 through January 15 for coverage starting February 1.

  1. Gather your details. Household size, estimated 2027 income, and current doctors or prescriptions.
  2. Check Medicaid first. If you may fall under 138% FPL, apply for NC Medicaid; it is free or low-cost and open year-round.
  3. Compare Marketplace plans. Apply your subsidy, then weigh premium, deductible, and network together.
  4. Enroll and confirm. Pay your first premium so coverage starts on time.

Missed the window? A special enrollment period may open if you lose coverage, move, marry, or have a baby. See our Open Enrollment 2027 guide for exact dates and triggers.

Frequently asked questions

Does North Carolina have its own health insurance exchange?

No. North Carolina uses the federal HealthCare.gov Marketplace. You enroll, compare plans, and apply subsidies through the federal platform rather than a state-run exchange.

Who qualifies for NC Medicaid after expansion?

Adults ages 19 to 64 who are North Carolina residents with household income at or below 138% of the federal poverty level generally qualify. There is no asset test, and Medicaid enrollment is open year-round. Starting January 1, 2027, most expansion adults must also meet a work or community-engagement requirement unless exempt.

How much does health insurance cost in North Carolina in 2027?

Prices are rising again: NC insurers requested 2027 increases of about 8.5% to 27.4%, with final rates set after state review. Most subsidized enrollees pay far less than the sticker price, because the premium tax credit caps what you pay for the benchmark Silver plan at a share of your income. Your actual cost depends on income, age, and county.

Did subsidies change for 2027?

The enhanced premium tax credits expired at the end of 2025 and were not extended, so for 2027 subsidies remain smaller and the 400% federal poverty level cliff still applies. Subsidies still exist, so it is worth checking your exact eligibility.

How do I get North Carolina health insurance quotes?

Enter your ZIP code, the ages of everyone who needs coverage, tobacco use and your expected 2027 household income on HealthCare.gov or with a licensed agent. Marketplace quotes never require health questions, and adding income shows your price after the premium tax credit.

What is the best individual health insurance in NC?

There is no single best plan. The best individual plan is the one whose network includes your doctors, whose drug list covers your prescriptions and whose total yearly cost fits your budget. At or below 250% of poverty, an enhanced Silver plan is often the best value.

Is short-term health insurance allowed in North Carolina?

Yes, but it is limited. North Carolina follows the federal definition, currently an initial term of up to three months and four months in total. Short-term plans aren’t ACA plans, can deny pre-existing conditions and can’t use premium tax credits.

Is Honesty licensed in North Carolina?

Yes. Honesty is licensed in North Carolina. Honesty.AI is a licensed, unbiased agent that compares plans across carriers, applies subsidies, and can enroll you, all while protecting your privacy.
