Health insurance Naples: why the city is different

Naples has an unusually old population, even for Florida. U.S. Census Bureau QuickFacts puts 56.2% of Naples residents at age 65 or older, compared with 34.4% across Collier County. Most of those residents are on Medicare, so the Marketplace questions here tend to cluster around the years just before and just after 65.

That shapes how to think about health insurance. Naples households often include an early retiree living on savings, a spouse a few years from Medicare, and adult children or grandchildren working in the service jobs that keep Southwest Florida running. Each needs a different answer. For statewide rules, start with our Florida health insurance guide.

Retiring before 65 in Naples

Leaving a job before Medicare age usually means leaving the employer plan too, and finding new health insurance. Naples early retirees have a few routes. You can buy a Marketplace plan during Open Enrollment, or within 60 days of losing job-based coverage through a Special Enrollment Period. COBRA is another option, but it doesn’t come with a premium tax credit.

The big variable for early retirees is income. The premium tax credit for 2027 is based on your expected 2027 household income, and for most people that means modified adjusted gross income. IRA and 401(k) withdrawals, capital gains, and Social Security benefits can all count. Because the enhanced credits expired at the end of 2025, the 400% “subsidy cliff” is back: one dollar over the line and the credit disappears.

Household size100% of poverty (subsidies start in Florida)400% of poverty (cliff for 2027 coverage)
1$15,960$63,840
2$21,640$86,560
3$27,320$109,280
4$33,000$132,000

Retiree tip: If your income is close to the 400% line, the timing of withdrawals or asset sales can decide whether you get a subsidy at all. Talk with your tax professional before you lock in a 2027 income estimate. Below 400%, you pay no more than 10.22% of income for the benchmark silver plan.

Turning 65: moving from the Marketplace to Medicare

The handoff from a Marketplace plan to Medicare is a common stumbling point. According to HealthCare.gov, Marketplace coverage doesn’t end automatically when Medicare starts. You have to update your Marketplace application to end it, and you can set the end date up to three months ahead.

  • Savings stop. Once you have Medicare Part A or Part C, you no longer qualify for help paying for a Marketplace plan.
  • Don’t delay Medicare. If you sign up late, you may wait until the General Enrollment Period (January 1 – March 31) and may owe lifelong late penalties for Part B and Part D.
  • Check start dates. Medicare.gov explains when coverage begins based on when you sign up.

The goal is a clean handoff: Medicare starts on the first day of a month, and your Marketplace plan ends the day before.

When one spouse reaches Medicare first

Many Naples couples are a few years apart in age. When the older spouse moves to Medicare, the younger one can keep or buy a Marketplace plan. Medicare is individual coverage, so it doesn’t cover a spouse.

The younger spouse’s subsidy still depends on total household income, including the Medicare spouse’s income, on a jointly filed return. If you are married, you generally must file jointly to claim the premium tax credit. Update the Marketplace application when the older spouse leaves so the household and coverage details stay accurate.

NCH hospitals and your network

NCH (Naples Comprehensive Health) is a locally governed nonprofit system based in Naples. It runs NCH Baker Hospital downtown and NCH North Hospital, with 713 beds in total, and lists dozens of locations across Southwest Florida.

Before you choose health insurance, Naples patients should confirm that NCH, or whichever hospital and specialists you rely on, is in the plan’s network. Search the plan’s provider directory for each doctor by name, then check the drug list for every prescription. Emergency care is covered anywhere, but planned surgery, cancer care, and specialist visits depend on the network. Our guide on how to choose a health plan explains HMOs, EPOs, and PPOs.

Insurer changes for 2027

Cigna is leaving the Florida Marketplace after December 31, 2026, and Sunshine State Health Plan’s Marketplace products are being discontinued. If you have either, your current plan lasts through December 31, 2026; pick a 2027 plan by December 15 to avoid a gap. Our Florida page has the details.

Premiums are rising again, too. KFF found insurers proposed a median 15% increase for 2027. Carriers and networks vary by county; compare every option on HealthCare.gov or with Honesty rather than renewing automatically.

Working families and the coverage gap

Not everyone shopping for health insurance, Naples included, is retired. Restaurant, hotel, landscaping, construction, and home-care workers often have no employer plan or one they can’t afford. For 2027, an employer plan counts as unaffordable if the employee’s share for self-only coverage is more than 10.22% of household income; in that case you may qualify for Marketplace savings.

Florida has not expanded Medicaid, so adults earning below 100% of poverty with no other eligibility can fall into the coverage gap. Apply for Medicaid through ACCESS Florida, and check Florida KidCare for children under 19. Every 2027 Marketplace plan caps in-network out-of-pocket costs at $12,000 for one person and $24,000 for a family.

Storm season and Special Enrollment

Collier County was approved for FEMA Individual Assistance after Hurricane Ian in 2022 under disaster DR-4673 (FEMA). Storms can cost jobs, force moves, and push seasonal residents to stay north longer, all of which affect coverage.

Losing coverage or moving permanently can open a 60-day Special Enrollment Period. If a FEMA-declared emergency kept you from enrolling, HealthCare.gov gives extra time after the incident period ends. Keep a photo of your insurance card and your plan’s member number on your phone before each hurricane season.

Does my Marketplace plan end automatically when I get Medicare?

No. HealthCare.gov says Marketplace coverage doesn’t end automatically when Medicare starts. Update your application to end it, and you can choose an end date up to three months ahead.
