Health Insurance in West Palm Beach, FL: 2027 Options, Safety Net & Help

Health insurance West Palm Beach residents buy on their own comes from the federal Marketplace at HealthCare.gov. Open Enrollment for 2027 coverage runs Nov 1, 2026 – Jan 15, 2027; enroll by Dec 15 for Jan 1 coverage. New for 2027, only certain immigration statuses qualify for savings, and uninsured residents can use Palm Beach County’s community health centers while they sort out a plan.

Health insurance West Palm Beach basics for 2027

West Palm Beach is the county seat of Palm Beach County, and like all of Florida it uses the federal Marketplace at HealthCare.gov for individual and family ACA plans. Every plan covers essential health benefits, and insurers cannot deny you or charge more for a pre-existing condition.

Dates: Open Enrollment runs November 1, 2026 through January 15, 2027. Enroll by December 15, 2026 for a January 1 start, or by January 15 for February 1. Treat December 15 as your real deadline. Details are in our Open Enrollment 2027 guide.

Costs: The enhanced tax credits expired on December 31, 2025, so subsidies are smaller and the 400% of poverty cliff is back for 2027. KFF reports insurers proposed a median 15% increase for 2027. If you have a plan now, re-shop instead of auto-renewing.

2027 change: immigration status and Marketplace savings

This is the biggest 2027 rule change for many West Palm Beach households. U.S. Census Bureau survey data shows that roughly three in ten West Palm Beach residents were born outside the U.S. (Census Reporter, West Palm Beach).

According to HealthCare.gov, starting January 1, 2027, the immigrants who may qualify for lower Marketplace premiums are:

  • Lawful permanent residents (green card holders)
  • Cuban or Haitian entrants
  • Compact of Free Association (COFA) migrants

Other lawfully present immigrants can generally still buy a Marketplace plan, but without savings. A mixed-status family can still apply for the members who qualify, and U.S.-citizen children may qualify for Florida KidCare. Applying for eligible relatives does not require you to apply for yourself.

Check before Open Enrollment: If your household received tax credits in 2026 under a status not on that list, confirm your 2027 eligibility now so you are not surprised by a full-price bill in January.

Self-employed, gig and hourly workers

If you drive, deliver, freelance, run a small business, or work hours that change with the season, your income is hard to predict, and your subsidy depends on it. For 2027 coverage, the Marketplace uses your estimated 2027 household income, and the credit is reconciled when you file your 2027 taxes.

Household100% FPL (subsidies start)400% FPL (no subsidy above)
1$15,960$63,840
2$21,640$86,560
3$27,320$109,280
4$33,000$132,000

Within that range, the IRS caps what you pay for the benchmark Silver plan at 2.15% to 10.22% of income, depending on where you fall. At or below 250% FPL, a Silver plan also lowers your deductible and copays. Report income changes during the year so your credit stays accurate. The coverage gap and Palm Beach County’s safety net

Florida has not expanded Medicaid, so most adults without children cannot get Medicaid at any income. Adults earning under 100% FPL often fall into the coverage gap: too little for a Marketplace subsidy, too much for Medicaid. Apply through ACCESS Florida anyway if you might qualify, and check kids for Florida KidCare.

If you are uninsured, Palm Beach County has a real safety net. The Health Care District of Palm Beach County runs the C. L. Brumback Primary Care Clinics, Federally Qualified Health Centers that welcome all patients regardless of ability to pay, with sites that include West Palm Beach, Mangonia Park and St. Ann Place (Health Care District of Palm Beach County).

One update: the District’s District Cares program is being discontinued and is no longer accepting new applications. It was never health insurance, so it is not a substitute for a Marketplace plan if you qualify for one.

The Florida Department of Health in Palm Beach County is headquartered downtown on Clematis Street and provides public health services such as immunizations, WIC and school health.

Hospitals and trauma care in West Palm Beach

West Palm Beach is home to St. Mary’s Medical Center, which includes Palm Beach Children’s Hospital, and Good Samaritan Medical Center, both part of Palm Beach Health Network. The Health Care District lists St. Mary’s as one of the county’s two Level I trauma centers, primarily serving northern Palm Beach County, alongside Delray Medical Center in the south (Palm Beach County trauma centers).

Emergency care is covered under every ACA plan, but the hospital you are admitted to afterward, and the specialists who follow up, depend on your network. Before choosing health insurance West Palm Beach plans offer, look up:

  • The hospitals closest to your home and work.
  • Your primary care doctor, any specialists, and your children’s pediatrician.
  • Your prescriptions on each plan’s drug list.

Carriers and networks vary by county, and 2027 plan details are not final until November 1. Compare every option on HealthCare.gov or with Honesty.

How to enroll, step by step

  1. Gather your details: household members, immigration documents if any, and your best 2027 income estimate.
  2. Apply once: the HealthCare.gov application checks you for tax credits and refers you for Medicaid or KidCare.
  3. Compare by total cost and network, not just the monthly premium. The 2027 out-of-pocket cap is $12,000 individual / $24,000 family at most. See how to choose a health plan.
  4. Enroll and pay your first premium to turn coverage on.

Frequently asked questions

Which immigrants can get Marketplace savings in West Palm Beach for 2027?

Starting January 1, 2027, HealthCare.gov says lawful permanent residents (green card holders), Cuban or Haitian entrants, and Compact of Free Association migrants may qualify for lower premiums. Other lawfully present immigrants can generally still buy a plan at full price. Citizen children in mixed-status families may qualify for Florida KidCare.

I am uninsured in West Palm Beach. Where can I see a doctor now?

The Health Care District of Palm Beach County runs the C. L. Brumback Primary Care Clinics, Federally Qualified Health Centers that welcome all patients regardless of ability to pay. Use them for care now, and check whether you qualify for a Marketplace plan, Medicaid, or KidCare.

Is the District Cares program still available?

No new applications are being accepted. The Health Care District of Palm Beach County is discontinuing District Cares, which was never health insurance. If you qualify, a Marketplace plan or Medicaid is the way to get real coverage.

I am self-employed. How do I estimate my 2027 income for a subsidy?

Use your expected 2027 net self-employment income plus other household income, based on last year’s tax return and any known changes. Update your application if your income changes during the year, because the credit is reconciled when you file your 2027 taxes.

Can I enroll after a hurricane if I lose coverage or have to move?

Often, yes. Losing other coverage or moving can open a Special Enrollment Period, generally 60 days, so you may not need to wait for Open Enrollment. Keep your insurance card, member ID and prescriptions saved on your phone during storm season.
