Health insurance Portland: four insurers for 2027
Portland spans Multnomah, Washington and Clackamas counties, and all three have four insurers selling individual plans for 2027, according to the Division of Financial Regulation’s county coverage chart. That is the most choice available anywhere in Oregon this year. Some central and eastern counties have only two.
The four companies are BridgeSpan, Kaiser Permanente, Moda Health and Regence BlueCross BlueShield of Oregon. More choice helps, but it doesn’t make the decision easier on its own. Kaiser plans mostly use Kaiser’s own clinics and hospitals, while the other three build broader networks. For health insurance, Portland shoppers should start with which doctors they want to keep.
Rates are higher again. DFR approved an average 21.6% increase for the individual market statewide. Your own cost depends on your tax credit, so compare plans with your savings applied. Our Oregon health insurance guide covers the statewide picture.
Buying on Explore Health instead of HealthCare.gov
Oregon moved off the federal platform for 2027. Portland residents now shop at Explore Health (ExploreHealthOR.gov), run by the Oregon Health Authority. It is the only place to get federal tax credits and cost-sharing reductions on an Oregon individual plan.
If you had a 2026 plan through HealthCare.gov, the state says you’ll receive information about renewing for 2027. Read it, then log in and update your income. A renewal carries over last year’s choices, but your premium, drug list and network can all change. Explore Health’s phone line is 855-268-3767, and its Local Help Finder lists community assisters and agents.
Providence Health Plan members: what changes
Providence Health Plan, based in Portland, is ending its individual and family plans after 2026. PacificSource is also leaving the individual market. Explore Health says it matched affected members with a plan from another insurer, which may be used for automatic re-enrollment if you don’t choose one yourself.
The insurance company is separate from the hospitals. According to OPB, Providence says closing the health plan leaves its hospitals and medical groups untouched. If you want to keep a Providence doctor, check which 2027 plans include them before you accept your matched plan.
Honesty.AI compares your matched plan with every other 2027 option and checks your doctors.
Portland hospital networks to check
Portland plans are often built around particular hospital systems. Two plans at a similar price can include very different doctors, so confirm your doctors, specialists and preferred hospital are in-network for 2027.
- OHSU, Oregon’s only academic health center, per OHSU. Many specialists and complex cases are treated here.
- Legacy Health, a nonprofit system with hospitals across the metro area.
- Providence, including Providence Portland and Providence St. Vincent medical centers.
- Kaiser Permanente, both an insurer and a care system, so its plans mostly use Kaiser facilities.
We are not ranking insurers or hospitals. Networks change every year. Search each plan’s provider directory by doctor name, and see our guide on how to choose a health plan for network types.
Kaiser or a broad network? A Portland-specific choice
Kaiser Permanente sells individual plans in only 11 Oregon counties, and the Portland metro is one of the places where you can choose it. That makes one question unique to health insurance Portland shoppers: do you want an integrated system or a broad network?
- Integrated (Kaiser). Your doctors, hospital, lab and pharmacy are usually in one system. Care is coordinated, but you generally need to use Kaiser facilities except in emergencies.
- Broad network (BridgeSpan, Moda, Regence). These plans contract with many independent doctors and hospital systems, such as OHSU, Legacy and Providence. Which ones are included differs by plan.
If you already have a doctor you trust, the answer is usually whichever plan keeps them. If you are starting fresh, think about which hospital you would want in a serious illness, then work backward. Health insurance Portland decisions made this way tend to hold up better than ones based on premium alone.
Multnomah County clinics and enrollment help
The Multnomah County Health Department runs primary care clinics that serve county residents, including people with low incomes and people who are underinsured. The county also helps residents sign up for private health insurance or the Oregon Health Plan. Start at the county’s primary care clinics page.
Central City Concern provides health care alongside housing, recovery and jobs programs, mainly for people facing homelessness or addiction. Federally qualified health centers across the metro offer sliding-fee care too.
Large hospital bills have a safety net. Oregon law requires nonprofit hospitals to offer financial assistance to patients with incomes up to 400% of the poverty level. Ask for the hospital’s financial assistance application before paying a large bill.
OHP, OHP Bridge and Healthier Oregon in Portland
Many Portland households qualify for public coverage that costs far less than a Marketplace plan. The Oregon Health Plan covers most adults up to about 138% of the poverty level, and OHP is open all year. OHP Bridge covers adults 19 to 64 with income between 133% and 200% of the poverty level.
Portland has large immigrant and refugee communities. Healthier Oregon gives full OHP benefits to people who meet income rules regardless of immigration status. Healthier Oregon members move to OHP Open Card in January 2027 with the same benefits. On the Marketplace side, federal tax credits from January 1, 2027 are limited to lawful permanent residents, Cuban and Haitian entrants and COFA migrants. Our 2027 subsidy guide explains the details.
Living in Vancouver, working in Portland
Thousands of people cross the Columbia River every day for work. Individual coverage follows where you live. If you live in Vancouver or elsewhere in Clark County, you shop on Washington Healthplanfinder, not Explore Health, even if your job and doctors are in Portland.
Washington’s open enrollment also runs to January 15, 2027, with its own state savings. Moving from one side of the river to the other is a qualifying event that can open a Special Enrollment Period. If your doctors are in Oregon, confirm that your Washington plan covers them. Kaiser Foundation Health Plan of the Northwest sells plans in both states.
