Health insurance Memphis basics for 2027
Memphis sits in Shelby County, and Tennessee has no state exchange, so Memphians who lack job coverage, Medicare or TennCare shop on HealthCare.gov. You can enroll there directly or through a licensed agent or assistant connected to it; the price of the same plan is identical either way.
Memphis is a logistics, health care and university town. Warehouse and delivery jobs with changing hours, hospital shift work, small businesses and gig driving all push people between job-based and individual coverage. When your hours drop or a job ends, a Marketplace plan with a premium tax credit is usually the first place to look for health insurance. Memphis workers who lose job coverage get 60 days to pick a new plan.
The Shelby County Health Department runs public health clinics, immunizations and flu shots, but it does not sell insurance. For the statewide rules, see our Tennessee health insurance guide.
Living near the Mississippi and Arkansas state lines
The Memphis area spreads across three states, and that changes how you shop. You buy Marketplace coverage in the state where you live, so a Southaven or West Memphis resident shops in Mississippi or Arkansas, not Tennessee, even if they work downtown.
The reverse matters too. Many Shelby County residents see doctors across the river or have family who do. Before you pick a 2027 plan, check whether its network includes any out-of-state doctors or hospitals you rely on. Emergency care is covered out of network, but routine care usually is not.
Regional One Health’s Elvis Presley Trauma Center describes itself as the only Level 1 trauma center within a 150-mile radius of Memphis, caring for patients from Tennessee, Mississippi, Arkansas and Missouri. That regional pull is a good reminder that Memphis networks are built for a whole tri-state area, and that comparing health insurance, Memphis families should look past the state line on a map.
Memphis hospital systems to check before you choose
When you compare health insurance, Memphis networks matter as much as the monthly premium. Two plans at the same metal level can include very different hospitals, so confirm your doctors and preferred hospital are in-network for 2027 before you enroll.
The systems Memphis shoppers ask about most include:
- Regional One Health, home of the Elvis Presley Trauma Center and a major teaching hospital.
- Methodist Le Bonheur Healthcare, including Le Bonheur Children’s Hospital.
- Baptist Memorial Health Care, with hospitals across the Mid-South.
St. Jude Children’s Research Hospital is different. St. Jude states that families never receive a bill from St. Jude for treatment, travel, housing or food (St. Jude). Your child still needs regular health insurance for everything else, so keep your family’s coverage active.
We do not rank carriers or hospitals. Carriers and networks vary by county and change every year; search each plan’s provider directory by name. Our checklist on how to choose a health plan explains HMO, EPO and PPO differences.

Health insurance Memphis enrollment dates for 2027
Shelby County follows the HealthCare.gov calendar. Open Enrollment for 2027 health insurance in Memphis runs November 1, 2026 through January 15, 2027, and December 15 is the date that matters for a January 1 start.
| If you enroll… | Your coverage starts |
|---|---|
| Nov 1 – Dec 15, 2026 | January 1, 2027 |
| Dec 16, 2026 – Jan 15, 2027 | February 1, 2027 |
| After Jan 15, 2027 | Only with a qualifying life event |
If you have a Cigna plan, act early: Cigna is leaving the individual market after December 31, 2026 and will not sell Tennessee Marketplace plans for 2027 (ACA Signups). Losing job coverage, moving or a new baby can open a Special Enrollment Period. Our Open Enrollment 2027 guide has the details.
Subsidies and the coverage gap in Shelby County
Because Tennessee did not expand Medicaid, the Marketplace premium tax credit in Memphis starts at 100% of the poverty level and ends above 400%. For one person that range is $15,960 to $63,840; for a family of four it is $33,000 to $132,000.
Within that range, the 2027 credit limits your share of the benchmark Silver premium to between 2.15% and 10.22% of income, depending on where you fall. If you earn under 250% of the poverty level, Silver plans with cost-sharing reductions can lower your deductible. Our 2027 subsidies guide walks through the math, and the IRS explains how the credit is reconciled at tax time.
Below 100%, most adults get no credit unless they fit a TennCare group such as pregnant women, parents or caretakers of a minor child, or SSI recipients. Apply at TennCare Connect. Children may qualify for CoverKids up to 250% of the poverty level. Our affordable health insurance Tennessee guide covers every income band.
Clinics and programs for uninsured Memphians
Memphis has an unusually strong network of community clinics for people in the coverage gap or between plans. None of them replaces health insurance; Memphis clinics can, however, keep primary care steady while you sort out coverage.
- Christ Community Health Services runs health centers across Memphis, welcomes patients regardless of insurance status, and offers a Sliding Fee Discount Program for medical, dental and behavioral health care based on household size and income.
- Memphis Health Center and Tri State Community Health Center are also federally qualified health centers with Memphis sites, per HRSA’s Find a Health Center data.
- Church Health offers the MEMPHIS Plan for uninsured people who work at least 20 hours a week in Shelby County and earn under 200% of the poverty level. Church Health describes it as filling the gap between TennCare and traditional insurance.
If you get hospital care without coverage, ask about financial assistance. Regional One Health posts its financial assistance policy online and lists a health insurance assistance service for patients.
Free in-person enrollment help in Shelby County
Shelby County is one of ten Tennessee counties where the state’s federal Navigator, Nashville-based Family & Children’s Service, offers in-person as well as virtual help for 2026–2027, according to the CMS Navigator awardee summary. Neighboring Fayette and Tipton counties are on the same in-person list. The summary names Memphis partners including Latino Memphis and the Shelby County school system.
Navigators are free and trained to stay neutral. Search your ZIP code at localhelp.healthcare.gov for an appointment. Spanish speakers can also apply at CuidadoDeSalud.gov; note that from January 1, 2027, Marketplace savings for immigrants are limited to green card holders, Cuban and Haitian entrants, and Compact of Free Association migrants.
Storm tip: Mid-South tornado and ice-storm seasons can knock out power and paperwork. Keep photos of every family member’s insurance card and your prescription list on your phone. If a federally declared disaster stops you from enrolling on time, you may qualify for a Special Enrollment Period.

Frequently asked questions
I live in Southaven but work in Memphis. Where do I buy coverage?
You buy Marketplace coverage in the state where you live, so a Southaven resident shops Mississippi plans on HealthCare.gov. Check that the plan’s network includes any Memphis doctors or hospitals you want to use.
Can I get in-person enrollment help in Memphis?
Yes. Shelby County is one of ten Tennessee counties with in-person Navigator help for 2026–2027 through Family & Children’s Service. Search your ZIP code at localhelp.healthcare.gov to book a free appointment.
Is the MEMPHIS Plan health insurance?
Church Health describes the MEMPHIS Plan as filling the gap between TennCare and traditional insurance for uninsured people who work at least 20 hours a week in Shelby County and earn under 200% of the poverty level. Compare it with a subsidized Marketplace plan if you qualify for both.
Does St. Jude replace my child’s health insurance?
No. St. Jude states that families never receive a bill from St. Jude for treatment, travel, housing or food, but that covers St. Jude care only. Your child still needs regular coverage for other doctors, hospitals and prescriptions.
When must Memphis residents enroll for January 1 coverage?
Enroll by December 15, 2026 for coverage starting January 1, 2027. Open Enrollment continues through January 15, 2027, with coverage starting February 1 for later sign-ups.
