Updated for the 2027 plan year (October 2026)

Health insurance Birmingham basics for 2027

Birmingham sits in Jefferson County and, like the rest of Alabama, uses the federal Marketplace. If you don’t have coverage through a job, Medicare or Medicaid, you compare and buy a plan on HealthCare.gov or through a licensed agent connected to it. The plan price is the same either way.

Alabama has not expanded Medicaid, so most adults qualify for help only once household income reaches 100% of the federal poverty level. From there up to 400%, a premium tax credit lowers your monthly cost. Our Alabama health insurance guide covers the statewide rules and the four insurers with approved 2027 rates.

The city’s job market shapes coverage too. UAB Medicine says the University of Alabama at Birmingham and its health system together are the largest employer in Alabama, with more than 23,000 employees. Many Birmingham households combine one job-based plan with a spouse or adult child who buys their own.

Job coverage, family members and the Marketplace

If you are offered a job-based plan, the Marketplace credit is usually off the table when the employee-only premium costs no more than 10.22% of household income in 2027. Since 2023, family members are judged separately: if adding a spouse or children to the job plan costs more than that share, they may qualify for Marketplace savings on their own, according to HealthCare.gov.

Children deserve a separate look. Alabama’s ALL Kids program covers children under 19 in families earning up to 312% of poverty, and it uses the Blue Cross and Blue Shield of Alabama network. For many families comparing health insurance, Birmingham parents end up with kids on ALL Kids and adults on a Marketplace or job plan.

UAB Medicine and other Birmingham hospital networks

Networks matter a great deal when you shop for health insurance in Birmingham. Two plans at a similar price can include very different hospitals and specialists, so confirm your doctors are in-network for 2027 before you choose.

  • UAB Hospital is the city’s academic medical center, with 1,207 licensed beds, according to UAB Medicine.
  • UAB St. Vincent’s, formerly an Ascension hospital, is now part of UAB Medicine. If your old insurance card or provider list says “Ascension St. Vincent’s,” check the current name in each plan’s directory.
  • Children’s of Alabama is the region’s pediatric hospital (childrensal.org). Parents should confirm it is in-network for their kids’ plan.
  • Brookwood Baptist Medical Center and Grandview Medical Center are other large Birmingham-area hospitals to look for in a plan’s directory.

We don’t rank insurers or hospitals. The right plan is the one whose network includes the care you already use, at a total yearly cost you can manage.

Cooper Green Mercy and Birmingham’s county safety net

Jefferson County has a resource most Alabama counties don’t: Cooper Green Mercy Health Services, a UAB affiliate whose mission is to serve Jefferson County residents regardless of ability to pay. It is at 1509 6th Avenue South.

To enroll in its Health 1st program, Cooper Green lists these requirements: age 18 or older, an Alabama resident for at least one year, and a Jefferson County resident for at least 30 days. Call 205-930-3377 or see the Cooper Green enrollment page for current income rules and documents.

In September 2026, UAB said Cooper Green added enrollment specialists in the emergency departments at UAB St. Vincent’s Birmingham and UAB St. Vincent’s East, so uninsured patients can be screened on the spot.

Know the difference: a local care program helps you get care at specific facilities. A Marketplace plan travels with you to any in-network provider, covers all essential benefits and caps your yearly out-of-pocket costs. If your income is 100% of poverty or more, compare a subsidized plan too.

2027 deadlines for Birmingham shoppers

Open Enrollment for 2027 coverage runs November 1, 2026 through January 15, 2027. Sign up by December 15 and your plan starts January 1. Sign up between December 16 and January 15 and it starts February 1.

After January 15, you need a qualifying life event, such as losing job coverage, moving or having a baby, to enroll. Our Open Enrollment 2027 guide lists the events that open a Special Enrollment Period.

Community health centers and free clinics in Birmingham

Federally qualified health centers serve patients with or without insurance and charge on a sliding scale by income. They are a good fit if you are uninsured, in the coverage gap, or want a primary-care home that accepts Marketplace plans.

  • Christ Health Center in Woodlawn, 5720 1st Avenue South.
  • Alabama Regional Medical Services (ARMS), Metro Health Center at 712 25th Street North.
  • Cahaba Medical Care in West End, 1308 Tuscaloosa Avenue SW.

All three appear as active health center sites in HRSA’s health center data. For people with no insurance at all, M-Power Ministries runs a free clinic at 4022 4th Avenue South.

The Jefferson County Department of Health runs three health centers, Central (Guy M. Tate), Eastern and Western, offering services such as family planning, pediatric care, dental, WIC and immunizations. They are not a substitute for a health plan, but they can fill gaps.

Paying for health insurance in Birmingham: 2027 subsidies

Your premium depends on age, ZIP code, household size and 2027 income. For 2027, the IRS sets what you pay for the benchmark Silver plan between 2.15% and 10.22% of income, depending on where you fall from 100% to 400% of poverty. Above 400%, there is no credit.

For one person, 100% of poverty is $15,960 and 400% is $63,840. If you earn up to 250% of poverty, Silver plans add cost-sharing reductions that lower deductibles and copays. The 2027 out-of-pocket maximum is $12,000 for one person. See our 2027 subsidies guide and the affordable health insurance Alabama hub for full tables.

Every Alabama insurer received an approved 2027 rate increase, so don’t auto-renew. Compare health insurance Birmingham plans side by side this fall, even if you liked last year’s.

Storm season and your Birmingham coverage

Central Alabama knows severe weather. The National Weather Service in Birmingham documented 62 confirmed tornadoes across Alabama in the April 27, 2011 outbreak, 29 of them in Central Alabama.

Keep a photo of your insurance card and plan ID on your phone, and know your plan’s urgent-care and telehealth options. If a disaster forces you to move or causes you to lose coverage, you may qualify for a Special Enrollment Period to enroll or change plans outside Open Enrollment.
