For health insurance in Rockford, residents use Get Covered Illinois, the state marketplace, not HealthCare.gov. Open Enrollment for 2027 coverage begins November 1, 2026. Premium tax credits help from 138% to 400% of poverty, and Illinois Medicaid covers many adults below 138%. Short-term plans can’t be sold in Illinois.
Updated for the 2027 plan year (October 2026)
Health insurance Rockford basics for 2027
Rockford is the county seat of Winnebago County, and its residents shop the same statewide exchange as the rest of Illinois. Get Covered Illinois took over from HealthCare.gov for the 2026 plan year and runs Open Enrollment again this fall.
Open Enrollment for 2027 coverage begins November 1, 2026. The exchange sets its own deadline and had not posted the 2027 end date by mid-October, so plan on at least January 15, 2027, and enroll by December 15, 2026 for a January 1 start. Our Open Enrollment 2027 guide explains how start dates work.
Two changes affect current members. Cigna and Molina plans leave Get Covered Illinois on January 1, 2027, per the exchange’s 2027 plan changes notice, and Illinois insurers proposed an average increase of about 14% for 2027. Compare plans instead of letting yours renew.
Living near the Wisconsin line
Rockford sits less than 20 miles south of the Wisconsin border, and plenty of families move or work across it. The two states enroll differently: Wisconsin uses HealthCare.gov, while Illinois residents must use Get Covered Illinois.
If you move from Wisconsin into the Rockford area, the move can open a special enrollment period, generally if you had qualifying coverage for at least one day in the 60 days before moving. A Wisconsin marketplace plan won’t carry over; you apply fresh in Illinois, and the reverse is true if you move north.
Networks matter here too. If you see doctors in Beloit or Janesville, check whether an Illinois plan includes them before you enroll, because many individual plans have networks built around one state.
Rockford’s hospital systems: check your network
Three health systems run the city’s main hospitals, and each health insurance Rockford plan may include one, two or all three. Before you pick a plan, confirm that your doctors and the hospital you would use are in-network.
- OSF Saint Anthony Medical Center on East State Street.
- UW Health SwedishAmerican Hospital, a division of UW Health that serves northern Illinois and southern Wisconsin.
- Mercyhealth Javon Bea Hospital–Riverside, a Level III perinatal center for the region, according to Mercyhealth.
If you are expecting a baby or managing a chronic condition, the hospital and specialists in a network can matter more than a small premium difference. Illinois also treats pregnancy as a qualifying life event, so you can enroll when you become pregnant.

Which health insurance Rockford path fits your income
Your household income decides where you land. The figures below use the 2026 poverty guidelines that apply to 2027 coverage, shown for a household of three.
| Income (% of poverty) | Household of 3 | Usual path |
|---|---|---|
| Up to 138% | Up to $37,702 | Medicaid for adults; All Kids for children |
| 138% – 250% | Above $37,702 | Marketplace plan with credit; Silver plans add cost-sharing reductions |
| 250% – 400% | Up to $109,280 | Marketplace plan with a smaller credit |
| Over 400% | Over $109,280 | Full-price plan; no credit in 2027 |
Children qualify for All Kids at higher incomes than adults, up to 318% of poverty on the Get Covered Illinois chart, so parents in a marketplace plan often have kids on All Kids. The affordable health insurance Illinois guide has the full income and contribution tables.
Starting with renewals in 2027, some Medicaid adults without children under 18 at home must meet new work, school or volunteer rules, and renewals move to every six months.
If your job offers coverage
Many Rockford-area workers get an offer from an employer, but the offer may not be the end of the story. For 2027, job coverage counts as affordable if the employee’s share for the lowest-cost self-only plan is 10.22% of household income or less.
If your own coverage passes that test, you can’t get a marketplace credit. Your spouse and children are judged separately: if adding them costs more than 10.22% of household income, they may qualify for a credit even though you don’t.
Contract, temp and part-time workers without an offer compare health insurance Rockford plans on the marketplace like anyone else. If your hours or pay swing, estimate your 2027 income carefully and update Get Covered Illinois when it changes, since the credit is reconciled on your 2027 tax return.
Community health center and county help in Rockford
Crusader Community Health is a federally funded Health Center Program grantee with sites in Rockford, Loves Park and Belvidere, plus school-based centers in South Beloit. It offers payment assistance for patients with limited income.
The Winnebago County Health Department provides public health services such as immunizations, STI care and tuberculosis care; its main line is 815-720-4000. Neither replaces insurance, but both can fill gaps while you sort out coverage.
If a hospital bill is already a problem, ask the hospital’s financial counselors about charity care before paying. Nonprofit hospitals must have a written financial assistance policy under federal tax rules, and you can ask for it.
Students and young adults
Rockford University, Rock Valley College and the University of Illinois College of Medicine Rockford bring a steady stream of students and young adults. Each age group has its own best option.
Under 26, you can usually stay on a parent’s plan, even if you live on your own. Under 30, you can also choose a catastrophic plan, though premium tax credits can’t be used on it. Students offered a school health plan can compare it with a marketplace plan; Illinois’ short-term ban does not stop student coverage written for less than a year.
Short-term plan ads Rockford shoppers see
National ads for short-term plans show up when people search for health insurance Rockford options, but insurers can’t sell those plans here. Since January 1, 2025, Illinois law has barred insurers from issuing, renewing or extending short-term, limited-duration plans for Illinois residents, according to the Illinois Department of Insurance.
Hospital indemnity and similar policies are still sold, but they pay set cash amounts and are not ACA coverage. If someone pitches a short-term plan to an Illinois resident, you can report it to the Department at 866-445-5364.

Frequently asked questions
Do Rockford residents use HealthCare.gov?
No. Illinois runs its own marketplace, Get Covered Illinois, at GetCovered.Illinois.gov. Nearby Wisconsin still uses HealthCare.gov, so people moving across the state line must apply in their new state.
Does moving from Wisconsin to Rockford let me enroll outside Open Enrollment?
Usually, yes. A permanent move can open a special enrollment period, generally if you had qualifying coverage for at least one day in the 60 days before the move.
Which hospitals should I check when comparing Rockford plans?
The main systems are OSF Saint Anthony Medical Center, UW Health SwedishAmerican Hospital and Mercyhealth Javon Bea Hospital-Riverside. Plans differ in which ones they include, so check the provider directory for your doctors and hospital.
Can my kids get All Kids if I buy a marketplace plan?
Often, yes. All Kids covers children at higher incomes than adult Medicaid, up to 318% of poverty, so parents in a marketplace plan frequently have children on All Kids.
Can I buy short-term health insurance in Rockford?
No. Since January 1, 2025, Illinois law bars insurers from issuing, renewing or extending short-term, limited-duration plans for Illinois residents, including people in Rockford.
