How health insurance in Houston works for 2027
If you do not get coverage through a job, Medicare or Medicaid, you shop for your own health insurance. Houston residents do that on the ACA Marketplace. Texas has no state exchange, so you use HealthCare.gov directly or enroll through a licensed agent connected to it. The plan price is identical either way.
Contractors, small-business owners, gig workers and people between jobs make up much of the individual market, and many Houston families move between job-based and individual coverage. The stakes are high: Texas had the highest uninsured rate of any state in 2024, at 16.7%, according to the U.S. Census Bureau.
Every Marketplace plan covers essential benefits such as doctor visits, hospital care, prescriptions, maternity and mental health, and no insurer can deny you for a pre-existing condition. Texas did not expand Medicaid, so most low-income adults without children cannot get Medicaid; children often qualify for Medicaid or CHIP at YourTexasBenefits.com. For the statewide picture, see our Texas health insurance guide.
Key 2027 dates for Houston shoppers
| If you enroll… | Your coverage starts |
|---|---|
| Nov 1 – Dec 15, 2026 | January 1, 2027 |
| Dec 16, 2026 – Jan 15, 2027 | February 1, 2027 |
| After Jan 15, 2027 | Only with a qualifying life event (Special Enrollment Period) |
Treat December 15, 2026 as your real deadline for New Year’s Day coverage. Losing job coverage, moving, marriage or a new baby can open a Special Enrollment Period outside these dates, generally for 60 days. More detail is in our Open Enrollment 2027 guide.
The Texas Medical Center and your plan’s network
When you compare health insurance, Houston networks matter as much as the premium. The city is home to the Texas Medical Center, which describes itself as the largest medical complex in the world and reports about 10 million patient encounters a year. Having world-class hospitals nearby does not mean every plan lets you use them.
Houston Marketplace plans are often built around specific systems. The names residents ask about most include Houston Methodist, Memorial Hermann, Harris Health (the county’s public system), and specialty centers inside the Medical Center. Two plans at a similar price can include very different doctors, so search each plan’s provider directory for your doctors, specialists and preferred hospital before you choose.
We are not ranking carriers or hospitals here. Networks change every plan year and 2027 county plan data is not final yet, so carriers and networks vary by county. Compare every option for your ZIP code on HealthCare.gov or with Honesty. Our checklist on how to choose a health plan explains HMO, EPO and PPO differences.
Harris Health financial assistance is not health insurance
A clinic discount is not the same as health insurance. Houston families rely on Harris Health’s clinics and hospitals. Its Financial Assistance Program is for households earning up to 150% of the federal poverty level, with sliding-scale help based on income. Harris Health states plainly that the program “is not an insurance plan.”
That matters for two reasons. The assistance generally applies to care at Harris Health, not to doctors and hospitals across the region. And it does not cap your costs the way a Marketplace plan’s out-of-pocket maximum does. If your household income is at or above 100% of the poverty level, check whether a subsidized plan gives you broader coverage for a similar monthly budget.
Honesty.AI checks networks, applies your subsidy and compares every plan in your Harris County ZIP code.
What you’ll pay: health insurance Houston subsidies by income
Your monthly cost depends on your age, household size, ZIP code and 2027 income. The premium tax credit limits what you pay for the benchmark Silver plan to a share of your income, from 2.15% at the low end to 10.22% between 300% and 400% of the poverty level, under the IRS rules for 2027 (IRS.gov). You can use the credit on any metal tier.
| Household size | Subsidies start (100% FPL) | Subsidy cliff (400% FPL) |
|---|---|---|
| 1 | $15,960 | $63,840 |
| 2 | $21,640 | $86,560 |
| 3 | $27,320 | $109,280 |
| 4 | $33,000 | $132,000 |
Above 400% there is no credit for 2027. Commission, overtime and contract income swings, so estimate your 2027 income honestly; the credit is reconciled on your tax return. Insurers also proposed a median increase of about 15% nationally for 2027 (KFF), after the enhanced credits expired at the end of 2025, so shop again even if you liked last year’s plan. Our 2027 subsidies guide has the full table.
Tip: If your income is under 250% of the poverty level, look at Silver first. Only Silver plans carry cost-sharing reductions, which lower your deductible and copays. The 2027 out-of-pocket maximum on any plan is $12,000 for one person.
Is your Houston plan leaving for 2027?
Two insurers are leaving the Texas individual market after December 31, 2026. Cigna is ending its ACA Marketplace plans in Texas and 10 other states (ACA Signups), and Baylor Scott & White Health Plan says on its member FAQ that it will discontinue Individual & Family Marketplace plans the same day.
If either company covers you, pick a new 2027 plan by December 15 rather than accept an automatic replacement that may not include your doctors. Bring a list of your prescriptions and providers when you compare.
Immigrant families in Houston: what changes in 2027
Many Houston households include people with different immigration statuses. Starting January 1, 2027, a 2025 federal law limits Marketplace savings to citizens, green card holders, Cuban and Haitian entrants, and Compact of Free Association migrants, according to HealthCare.gov.
Other lawfully present immigrants may still buy a plan but cannot get the premium tax credit. Citizen children and other eligible relatives keep their own savings, Medicaid or CHIP. Spanish speakers can apply at CuidadoDeSalud.gov. Read more in our guide to the 2027 immigrant rules.

Community health centers
For sliding-fee primary care, Legacy Community Health describes itself as the largest federally qualified health center in Texas, with dozens of Houston-area sites listed in HRSA’s Find a Health Center directory. The Houston Health Department and Harris County Public Health run immunizations and other public health services.
Hurricanes, heat and your Houston health plan
Storm season is part of owning health insurance. Houston sits close enough to the Gulf to feel every major storm. When Hurricane Beryl came ashore near Matagorda on July 8, 2024, its center passed over the western side of the Houston metro area. The National Hurricane Center’s Beryl report notes about 2.7 million people lost power in southeastern Texas, and 14 deaths were tied to heat during those outages.
Keep photos of each family member’s insurance card, your plan ID and your prescription list on your phone. If you evacuate, emergency care is covered even outside your network. Losing coverage or moving after a disaster can open a Special Enrollment Period, so call the Marketplace or ask Honesty if a storm disrupts your enrollment.

Frequently asked questions
Where do I buy health insurance in Houston?
Individual and family plans in Houston are sold through the federal Marketplace at HealthCare.gov, or through a licensed agent connected to it. Texas has no state exchange. The same plan costs the same whichever way you enroll.
Is Harris Health’s financial assistance the same as health insurance?
No. Harris Health says its Financial Assistance Program is not an insurance plan. It helps households up to 150% of the poverty level with care at Harris Health, but it does not replace a Marketplace plan.
Can I get free in-person enrollment help in Houston?
Yes. For 2026–2027, Civic Heart Community Services and Light & Salt Association are federally funded Navigators with in-person help in Harris County. Search your ZIP code at localhelp.healthcare.gov to find an appointment.
Can a hurricane let me enroll outside Open Enrollment?
Sometimes. Losing coverage or moving after a storm can qualify you for a Special Enrollment Period, and CMS has offered disaster flexibilities in past emergencies. Contact the Marketplace promptly if a storm disrupts your coverage or enrollment.
Do Houston plans include the Texas Medical Center hospitals?
It depends on the plan. Marketplace networks are often built around specific hospital systems, so a plan may include one Medical Center hospital and not another. Search each plan’s provider directory for your doctors and hospital before you enroll.
